Commitment to Privacy
The appropriate collection, use, and disclosure of patients’ personal health information is important to us at Skilled Lactation Solutions, LLC. We commit to protecting the privacy and confidentiality of our clients’ information. This policy was last modified on the 12th of January, 2021. From time to time we may update this policy and make those changes available on our website.
Collection of Information
We collect information about you and your family that is important to provide you with the best care and to manage our practice. Information is stored on our computers and our EHR (electronic health records). Information is mostly collected electronically. Any information collected over the phone will be recorded electronically, in our EHR.
Use and Disclosure
Your health information will be treated as confidential information and used for the direct purpose of your care. There are circumstances where we may be permitted or required by law to disclose your personal information to third parties. For example, insurers, police, government regulatory bodies, tribunals, or courts of law.
Information collected for purposes other than medical care, such as support group meetings or media release contracts, are stored in our GSuite and are password protected. Only basic demographic and contact information are used for this purpose.
Reasonable steps will be taken to secure your personal health information including some of the following:
- Electronic information is stored in our secure, HIPAA compliant EHR software (Intakeq).
- Personal devices are password protected and not shared.
- Physical copies of protected health information are not created.
- If any breach is suspected to have occurred and involved your information, you will be notified in a timely manner.
You are entitled to your own medical records. If you wish to receive copies of your medical records, we require a request via email sent to firstname.lastname@example.org. We will provide you with copies of your medical records within 3 business days.
